"We will develop a care plan to accommodate the special needs of each senior their family member and their caregiver"
The staff at Soka Services is experts in developing care plans to assist seniors with their needs. We work at helping the older adult, their family and caregiver to realize a sense of empowerment over their day to day routines. We want to develop a relationship of advisor and advocate, linking seniors, family and caregivers to much needed community resources. The staff continues with case management to ensure that community resources make contact with the senior, family or caregiver and our clients receive the best possible care.
